When a loved one lives in assisted living or memory care, families need more than occasional updates. Care Advocacy AZ becomes a consistent, independent local presence for authorized Phoenix-area families. We begin by organizing the care story, medications, providers, preferences, recent events, and the details that define the resident’s usual baseline.

Authorized wellness visits occur at varied times and focus on observable changes in comfort, mobility, communication, engagement, environment, and daily routine. Families receive a clear summary with open questions and tracked next steps. Our field observations do not replace facility care or medical treatment. When the agreed escalation criteria are met, an RN reviews the concern and our Nurse Practitioner clinical lead supports the defined communication pathway.

This service is designed for local and out-of-state families who want dependable visibility, organized communication, and fewer gaps between the facility, providers, hospital, and family. Service availability depends on authorization, geography, and the initial Care Advocacy Assessment.
