How to prepare for an assisted living wellness visit.

A useful wellness visit starts with a clear picture of the resident’s usual routine. Before the visit, gather the current medication list, provider names, recent appointment notes, mobility level, diet, sleep pattern, communication preferences, and any changes the family or facility has noticed. Write down two or three questions that matter most.

During the visit, compare what you see with the established baseline. Notice alertness, breathing, movement, grooming, hydration, appetite, mood, social engagement, the condition of the room, and whether needed items are accessible. Ask staff for factual updates and document who provided the information. Avoid diagnosing or changing treatment; clinical concerns belong with the treating team.

Afterward, summarize observations in plain language, separate facts from questions, identify unresolved items, and confirm who will follow up. Care Advocacy AZ provides authorized varied-time wellness visits, organized family reports, RN review, and triggered NP leadership when the agreed escalation pathway calls for it.
