This secure resident profile gathers the information Care Advocacy AZ needs to understand the person behind the care plan: preferred name, routines, communication style, mobility and sensory needs, current living setting, important relationships, care-team contacts, baseline abilities, recent changes, and family priorities. Medical records, diagnoses, medication lists, insurance documents, photographs, and legal documents must not be submitted through ordinary Wix forms or email. The complete resident profile will be collected through IntakeQ/PracticeQ after an introductory conversation and authorization review. The secure portal link will be added here once the replacement forms are activated.
